In consideration of my child being allowed to participate in the OYFL program, I hereby release & hold harmless the OYFL, coaches, members, & volunteers, from any and all liability for injuries, damages, medical expenses, or any other loss to my child/family/self (including attorney's fees) arising from or relating to my child's participation in the OYFL activities. The price is for a single player if you have additional family members signing up you will have to fill out another signup form please note there is a discount if you sign up an additional family member up to three then if there's a fourth family member they play for free. Discounts will be received when picking up your raffle tickets please let an OYFL staff know you have a discount coming for signing up more than one family member.All payments are final you will be issued raffle tickets in the amount of payment to keep or sell to offset signup costs.
